Android 21's insatiable hunger is caused by her complex bio-android biology, specifically the combination of Majin Buu's cells and Saiyan DNA Because her hunger is so extreme, it triggered the creation of a malicious, cold-hearted split personality. The evil half actively craves the energy of the strongest warriors in the universe and takes over completely whenever her body's energy runs low or her cells go berserk.
