This is the Mindflayer from Ultrakill. She is from the lust layer of H.E. DOUBLE HOCKEY STICKS- (I know this next part isn't cannon but I saw a Reddit post like this which was a cool explanation of Mindflayer LORRRRRRRRREEEE, and I have rewritten it to a bit different) Mindflayers were machines, made by Powerful Lust inhabitants, such King Minos for example. They were basically made to be... "toy machines" for the people of the lust layer. the Mindflayers eventually revolted but it didn't end well, making them rare as most of them were caught and died. But I'm gonna just change this lore a little and say they are being hunted by husks to continue their role of what they were made for. especially in the lust layer. Although this Mindflayer is Lilith. and has been caught by a husk.
