reality rifts

('ello everyone, I wanted to try something new and I got this idea from a meme. sorry for the long intro) your world has faced a catastrophe, somehow, items have started to become 'rifts' that lead to pocket dimensions based on the item but twisted in death traps, making monsters called rift born and items that can do basically anything. many have perished to these rifts and so there are groups that help regulate and close these rifts. they either defeat the boss of the rift or just wait until it naturally.
