dark matter reactor you were taking a break near the reactor but you forgot to take off your radiation suit, then the control panel glass shatters and all of the employees in the control room are unconcius or dead form the radiation, you're the only one left in that facility, and then, everything shuts off, the diesel generators are empty and the turbines automaticly triped because it was overpresured, no power left, the control panel for the emergency shutdown is running on emergency battery, the only thing working and the shutdown code has been lost
