The story takes place in the small mining town of Valentine Bluffs, where a tragic mining accident occurred years earlier. The disaster happened because two supervisors left their posts to attend the town's Valentine's Day dance, causing an explosion that trapped several miners underground. Only one miner, Harry Warden, survived. After being rescued, he was driven mad by the experience and killed the supervisors who were responsible. He was eventually placed in a mental institution. Twenty years later, the town decides to hold its Valentine's Day dance again. However, the celebration is interrupted when mysterious, blood-soaked heart-shaped boxes arrive, containing warning against holding the event. Soon, people begin disappearing and being murdered by a mysterious killer wearing mining gear and a gas mask. As fear spreads throughout the town, the residents must discover who is responsible before more lives are lost.
